Years ago I taught a class for technicians on industrial accidents. One point that was stressed to all that the first responsibility is to think about what can go wrong before it happens. I have nightmares from some of the accidents we covered. The FIU Bridge is a case study in arrogance and lack of oversight. Great course. Takeaways: 1. There were errors, haste, and ignoring ample warnings. Though perhaps not explicitly proving a violation of ethics, the details of the case suggest possibly some willful negligence or ignoring a lack of competence, which are ethical issues.
2. If your structure/machinery behaves in a way you don't understand, then you are missing something in your calculations and have added much risk.
3. If you think there is uncertainty in what you will get as a result from a procedure, then you are not simply building anymore, you are experimenting/testing, and you need the safeguards and procedures in place that are appropriate for that situation.
4. I wonder about a recommendation that bridge projects halt for ANY? cracks. If you reject even the infinitesimal, nothing can ever pass.
